What is Self-Exclusion?

  • Self-exclusion is a tool that allows a person to request to be banned from either all Arizona Tribal Gaming Facilities or Event Wagering & Fantasy Sports (EWFS) Contests within the State of Arizona. If preferred, individuals may opt to self-exclude from both tribal casinos and EWFS contests. Both exclusions provide that a person on the self-exclusion list will be prohibited from collecting any winnings or recovering any losses. This self-ban includes the use of any of the services or privileges of the facility, such as restaurants, concerts, conventions, and hotels. The time period options available to Arizonans are either a one-year, five-year, or ten-year exclusion. Please note: the self-exclusion is irrevocable and cannot be altered or rescinded for any reason during the time period selected on the form.
  • The Arizona Department of Gaming offers two types of self-exclusions: Casino self-exclusion - where you can choose to exclude yourself from gambling at all Arizona casinos, as well as EWFS self-exclusion - where you can choose to voluntarily exclude yourself from placing wagers or purchasing fantasy sports contest entries at any licensed facility or on any internet/mobile platform. 
  • These exclusions are separate; if you exclude yourself from Arizona casinos, you are not excluded from EWFS and vice versa. However, you can choose to complete both self-exclusions if preferred. How Do I Exclude Myself From Arizona Casinos or Event Wagering & Fantasy Sports Contests?

  • There are several ways you can go about excluding yourself. The forms (available in a fillable PDF) can be found in three separate sections of this website: on the DPG home page, the the navigation bar, and the Resources page. The Event Wagering & Fantasy Sports Contests specific self-exclusion form can also be found on the EWFS tab.
  • You can download the appropriate self-exclusion form on this webpage, fill it out in its entirety, have it notarized, and then mail it to the Arizona Department of Gaming along with a current photo of yourself in color ink. Please note: the self-exclusion will not be processed without proper notarization and a current color photo. 
  • The Department is willing and able to provide notary services to those who request a notary. Photos may be submitted electronically via email; however, the original, notarized self-exclusion form must be mailed or delivered directly to the Arizona Department of Gaming’s main office: 100 N. 15th Ave., Suite 202, Phoenix, Arizona 85007. If submitted online, color pictures can be emailed and/or attached to the form.
  • In-person self-exclusions are currently available Monday through Friday from 8:30 AM until 4:00 PM. Please note that you can choose to call and schedule an appointment, and we will assist you by walking you through the form and notarizing the self-exclusion for you, as well as taking your photo, all in the same day. Please do not let this time frame stop you from excluding yourself or receiving the help you may need, as we can be flexible and adjust if necessary.
  • The Arizona Department of Gaming has recently added a remote notary option as another option to notarize the self-exclusion form. Please review the FAQ list to see what technology requirements are required and the acceptable forms of identification for the remote notary option.
